Introduction

Maxillary Skeletal Expander (MSE) or Miniscrew-Assisted Rapid Palatal Expansion (MARPE) is a non-surgical orthodontic treatment that uses mini-screws for anchorage to widen the upper jaw (maxilla) in both adolescents and adults, even after the midpalatal suture has fused, offering a less invasive alternative to surgery (SARPE). By applying force directly to the bone via the screws, MARPE achieves greater skeletal expansion, creates space for crowded teeth, improves nasal airflow, and reduces unwanted tooth movement or dental side effects, leading to better bite and airway function.

How MSE/MARPE Works

Unlike traditional expanders that anchor only to the teeth, MSE/MARPE utilizes small, biocompatible miniscrews (Temporary Anchorage Devices or TADs).

Appliance Placement

A custom-made expander with miniscrews is placed in the palate, with screws anchored directly into the bone.

Activation

A key is used to turn a screw in the device, typically daily for 4-6 weeks, gradually pushing the two halves of the upper jaw apart.

Bone Growth: This process splits the midpalatal suture, and new bone forms to maintain the expanded space.

Retention

The expander stays in place for several months (4-6) to allow bone consolidation. TADs are primarily made from biocompatible titanium or titanium alloy, which the body readily accepts.

Adults and Older Teens

Those whose jaw growth has stabilized.

Respiratory Issues

Patients struggling with nasal obstruction or sleep-disordered breathing.
